My role is focused on practical support. I help clients and families navigate community resources, referrals, care coordination, and the non-clinical pieces that can make it easier to follow through on a plan.
I may help identify local resources, clarify what information a program needs, or connect clients back to the right person on the Mesa & Mind team. Case management is not therapy, but it can make the rest of care feel more manageable.
